Scientific Studies on Tadalafil
“A Randomized, Double-Blind, Placebo-Controlled, Parallel Study to Assess the Efficacy and Safety of Once-A-Day Tadalafil in Men with Erectile Dysfunction Who Are Naïve to PDE5 Inhibitors” by Francesco Montorsi et al discussed the use of tadalafil in men with erectile dysfunction when given once on a daily basis. The reason behind conducting this research was that the trials that were held previously in this regard were all comprised of patients which were not naïve to the use of tadalafil and a study comprising of people who were naïve to therapy with tadalafil was highly suggested. Hence, the safety and effectiveness of tadalafil were to be measured in non-naïve persons. The main parameters of checking the efficacy of tadalafil were the International Index of Erectile Function (IIEF) and the responses to the question 2 and question 3 of the Sexual Encounter Profile (SEP). 217 people were selected for this study who were suffering from erectile dysfunction and were naïve to the use of PDE5 inhibitors. One third of these people were subjected to receive placebo during this study while the other two third of the patients were given tadalafil in a dose of 5 mg on a daily basis. This therapy continued for a time period of 12 weeks. Results were recorded according to the set parameters at the end of the therapy and it was seen that the people who were given tadalafil showed more improvements in their IIEF and SEP scores as compared to those shown by the people receiving placebo. People who used tadalafil experienced from some common side effects that included myalgia, back pain, headache, dyspepsia and nasopharyngitis. Moreover, four of the subjects who received tadalafil and one who was given placebo had to withdraw because of these side effects. It was concluded that people receiving tadalafil once a day and were naïve to its use showed improvements in their erectile function. 
“Tadalafil monotherapy and as an add-on to background bosentan in patients with pulmonary arterial hypertension” by Robyn J. Barst et al focused on the use of tadalafil in patients who were using bosentan in order to treat their pulmonary arterial hypertension. Tadalafil has been regarded as an effective agent in the treatment of pulmonary arterial hypertension when taken in a dose of 40 mg. This study was carried out to evaluate its effectiveness as well as safety in people who were naïve to the treatment and were going to use tadalafil in addition to bosentan which they were using already for their hypertension. Various groups were analyzed which included the group receiving placebo who was naïve to the treatment, the group naïve to the treatment and receiving tadalafil, a group who received placebo in addition to bosentan and finally the last group who received a combination of bosentan and tadalafil. These people were assessed for side effects, clinical worsening, and a six-minute walk distance. The patients were checked for any alteration in the above-mentioned parameters by the administration of the 4 different drugs in the four specified groups. After careful evaluation of the data, it was proven that the administration of tadalafil in the patients suffering from pulmonary arterial hypertension was tolerated to a great extent in the patients who were naïve to this treatment. Also, it was tolerated and produced improvements when taken along with bosentan but no additional effect, in this case, could be specified. 
How Tadalafil and Dapoxetine Work
Tadalafil is a PDE5 inhibitor and it works by increasing the levels of cyclic guanosine monophosphate in the muscles of the penis. It actively inhibits the enzyme called phosphodiesterase 5 which in normal cases break down the cGMP in the corpus cavernosum of the penis. By preventing this breakdown of the cGMP, it accumulates in the penile muscles and this consequently results in relaxation of these smooth muscles. It also causes dilation of the penile vessels which encourages more blood to flow towards the penis. These effects combine together to help the patient using tadalafil gain a hard erection and also helps to maintain it for a long period of time. Dapoxetine is an agent that works mainly in the brain to relieve the patients of the symptoms of depression associated with erectile dysfunction. It stimulates the secretion of serotonin in the brain which helps to improve the depressive mood of the patient and also help him to overcome the problem of premature ejaculation.

When Combined with Other Drugs
TD pills should never be used along with nitrates in any form including nitroglycerin, nitroprusside, etc because this combination can cause a strong hypotensive effect on the body of the patient. Alpha blockers such as tamsulosin must also not be administered along with TD pills as they can cross react to reduce the BP of the patient to dangerous levels. Similarly, the drug has a potential to cross-react with other antihypertensive drugs and alcohol as well. Antacids can also cross react with tadalafil in order to reduce their rate of absorption.
