“A 6-month study of the efficacy and safety of tadalafil in erectile dysfunction treatment: A randomised, double-blind, parallel-group, placebo-controlled study in Australian men” by C.G. McMahon et al is a research study that focused on tadalafil role in erectile dysfunction treatment in Australian men. It was a randomized placebo-controlled study that was double-blind and was set to be carried out for a time period of six months. 93 men from Australia were selected for this study who were suffering from erectile dysfunction of any severity i.e. mild, moderate or severe which was due to psychogenic factors, organic factors or a combination of different causes. These men were set to be given tadalafil in a dose of 20 mg according to their needs. 47 such patients were included in another group which was given placebo during the entire time of therapy. Sexual Encounter Profile (SEP) and International Index of Erectile Function (IIEF) were the parameters on the basis of which it was to be decided whether tadalafil proved to be of any significance in this disease. It was observed that the patients who received tadalafil during the course of therapy showed significant improvements in their sexual health as compared to the placebo group. At the end of therapy, question 3 of SEP diary was assessed which showed that the average of patients who were given tadalafil for 6 months and who could successfully carry out an intercourse was 73.5 percent. This rate was quite low among the group that received placebo i.e. 26.8 percent. 78 percent of people who were given tadalafil additionally reported a visible improvement in the erections as compared to 12.8 percent of the people who were given placebo treatment. Dyspepsia and a headache were considered to be the most common side effects seen during the therapy and were shown to be mild to moderate in their severity. The placebo-treated group lagged far behind standing still at a score of 14.9. 50 percent of the people who received 2.5 mg of tadalafil experienced successful intercourse while the rate of successful attempts in patients receiving 5 mg was found to be 62 percent. The efficiency of tadalafil was also found to be consistent with open-label extension of these studies in the patients. In patients suffering from diabetes and ED, this rate of successful attempts was found to be 40 percent in people using tadalafil in any dose. The prevalence of side effects in this study was found to be same as that in the on –demand use of tadalafil. It was concluded in this study that a possible balance of risk and benefit in using tadalafil in doses as low as 2.5 mg or 5 mg exists. 
Mode of Tadalafil Working
Tadalafil works to treat erectile dysfunction due to its inhibitory effect on the PDE5 enzyme present in patients’ penis. This enzyme normally cleaves cGMP levels in the smooth muscles of the penis not allowing it to accumulate.
